When is Surgery Necessary?
Glenoid bone grafting is usually considered when:
Recurrent Shoulder Instability with Bone Loss: When repeated shoulder dislocations have resulted in significant bone loss on the glenoid.
Failed Previous Stabilization Procedures: When prior surgeries to stabilize the shoulder have not been successful due to glenoid bone deficiency.
Large Glenoid Defects: When imaging studies reveal a substantial amount of missing bone on the glenoid.
Revision Surgery: When a previous shoulder surgery needs to be redone and bone loss is an issue.
What happens during the procedure?
Anesthesia: General anesthesia is typically used.
Incision: An incision is made over the shoulder joint.
Bone Graft Preparation: The bone graft is harvested or prepared.
Graft Placement: The bone graft is placed onto the deficient area of the glenoid.
Fixation: The graft is secured with screws, plates, or sutures.
Closure: The incision is closed with stitches or staples.
Types of Bone Grafts:
Autograft: Bone taken from another part of the patient's body (e.g., iliac crest, distal tibia).
Allograft: Bone obtained from a deceased donor.
Coracoid Transfer (Latarjet): While also a stabilization procedure, it also adds bone to the glenoid.
Recovery
Sling: A sling is typically worn for several weeks.
Physical Therapy: Rehabilitation is crucial for regaining shoulder strength and range of motion.
Pain Management: Pain medication is prescribed as needed.
Activity Restrictions: Certain activities are restricted for several months.
