What is Capsular Release?
Capsular release is a surgical procedure performed to treat frozen shoulder (adhesive capsulitis), a condition characterized by stiffness and pain in the shoulder joint. Frozen shoulder occurs when the capsule surrounding the shoulder joint thickens and tightens, restricting movement. A capsular release aims to loosen or cut this tightened capsule to restore range of motion.
When is Surgery Necessary?
Surgery is typically considered when:
Persistent Symptoms: When conservative treatments (physical therapy, pain medication, corticosteroid injections) have failed to provide significant improvement after a prolonged period (typically 6-12 months).
Severe Functional Limitation: When the stiffness and pain significantly impact daily activities and quality of life.
Chronic Frozen Shoulder: When the frozen shoulder has progressed to a chronic stage and is unresponsive to non-surgical interventions.
Types of Capsular Release
Arthroscopic Capsular Release: This is the most common method, performed with small incisions and an arthroscope to visualize and release the tightened capsule.
Manipulation Under Anesthesia (MUA): While not a direct "release" in the surgical sense, MUA is a procedure where the shoulder is forcefully moved through its range of motion while the patient is under anesthesia, to break up adhesions. This is less invasive than surgery, but can be painful.
Surgical Procedure (Arthroscopic)
Anesthesia: General or regional anesthesia is used.
Incisions: Several small incisions are made around the shoulder.
Visualization: An arthroscope is inserted to visualize the joint capsule.
Capsule Release: Specialized instruments are used to cut or release the tightened portions of the capsule.
Closure: The incisions are closed with stitches or adhesive strips.
Recovery
Sling: A sling is typically worn for several weeks.
Physical Therapy: Rehabilitation is crucial for regaining shoulder strength and range of motion.
